Military Family Doctor Network

Calian Health, in partnership with Military Family Services (MFS), is pleased to offer the Calian Military Family Doctor Network (MFDN) as part of its corporate citizenship efforts. The goal of the program is to improve access to quality health care for families of serving Canadian Armed Forces members by expediting the process of obtaining a physician and reducing the number of military spouses and children without a family physician.

The program is provided at no cost to families of the Canadian Armed Forces and is now available in Calgary, Comox, Edmonton, Esquimalt, Gagetown, Halifax, Kingston, London and SW Ontario, Mainland BC, Moose Jaw, St. John’s, Toronto, Ottawa, Trenton and Winnipeg. There are a number of family physicians at Primacy clinics across these communities who are participating in the MFDN program.  Calian staff have also started to appeal to family physicians who are not practising at Primacy clinics to make room in their practices for military family members. Locations will be added regularly, so be sure to check back for a location near you.

How to Register

You can access the program in one of the following ways:

  2. Print out the online form and fax it to 1-888-511-0219
  3. Call Calian’s team at 1-877-633-7722 x 3507

*Please download and save the form under a unique file name prior to filling it out. Once completed, click on the Submit completed form button at the bottom of the document or attach the form to an email and email it to [email protected].

A representative from Calian’s team will be in touch to review your requirements and provide you with more information about primary care options in your community.

Please note that the Military Family Doctor Network is not an urgent referral service. For emergencies, please seek care in your community or call 911.

The Military Family Doctor Network does not manage or oversee participating clinics and their practices. Clinics and physicians are subject to oversight by the regulatory bodies in their provinces.
