The Carlton & Church Medical & Walk-In Clinic operates as a walk-in clinic in Toronto, ON

Our facility staffed by board certified emergency and family physicians is here to help patients with their medical illnesses, injuries and questions both during office and after hours, thereby preventing patients from spending unnecessary time waiting in overcrowded Emergency departments and easing the burden on primary care providers after hours and on weekends.

We do not release medical information over the phone – NO EXCEPTIONS!

Sports Medicine

Referral from family doctor / walk-in physician isrequiredto be seen.
Please arrive at least 15 minutes prior to your appointment to fill out paperwork and/or to change into shorts, tank top, etc.
Shorts are required for all back pain and lower limb referrals.
Tank tops are required for al neck and upper limb referrals.
A minimum of 24 hours notice is required for all cancellations without penalty.
No-show fees will apply for all booked appointments.
If arriving late to a booked appointment, the full appointment time may not be available and you may be required to re-book your appointment.
Please note: the clinic does not treat patients with workplace or motor vehicle injuries (we reserve the right to cease treatment).
